Privacy Notice Of Ivy Dental
  1. These Terms are between you, the Patient, and Ivy Dental.
  2. This is to be read in conjunction with the current Dental Membership promotional literature available from Ivy Dental.
  3. The Dental Membership is a pre-payment scheme to spread the cost of your ongoing preventative dental care on a rolling basis, and to provide additional discounts and benefits to Members as described in the current Dental Membership promotional literature.
  4. Dental care will be provided at Ivy Dental at the appropriate intervals throughout the year as per the Membership level you have joined.
  5. Payment will be collected by Direct Debit. Payments will be collected until you cancel your membership in line with the cancellation notice required.
  6. Direct Debit payments will be collected on or around the 1st of the month. Should a collection fail for any reason, we will represent and attempt to collect payment again 10 days later.
  7. Membership will commence on receipt of the first payment, unless otherwise agreed with Ivy Dental.
  8. You can cancel your Membership by giving a minimum one months’ notice in writing to Ivy Dental outside of any initial term.
  9. The initial term of your Membership is 12 months from the date of joining.
  10. Should you cancel your Membership outside of any initial term, Ivy Dental reserves the right to recoup any discounts applied during this period and/or other outstanding payments due.
  11. It is your responsibility to attend at the agreed frequency as per your Membership level. Failure to do so will not result in any refunds being issued and/or Ivy Dental reserves the right to apply a failure to attend fee.
  12. Ivy Dental will send remainders of your appointments by text or email, however cannot guarantee the arrival of such messages. You are responsible for ensuring that your contact details are kept up to date and scheduling your appointments.
  13. Ivy Dental requires 2 working days’ notice should you need to reschedule appointments otherwise we reserve the right to charge a short notice cancellation fee.
  14. Ivy Dental may increase the cost of your Membership at any time by giving a minimum of 10 days’ notice, but usually one months’ notice. Ivy Dental will typically review Membership pricing once per annum.
  15. Any communication relating to details or changes pertaining to your Membership will be sent by email where possible, and post otherwise.

THE IVY DENTAL PRACTICE LIMITED Financial Services Register number 999664 introduces patients to credit provided by third-party lenders. The credit broking activity is carried out by Denplan Limited trading as Denplan (company number 1981238, Financial Services Register number 195821), which is an Appointed Representative of Simplyhealth Funding Services Limited and acts as a credit broker, not a lender. Credit is arranged by Zopa Bank Limited trading as DivideBuy (company number 10627575, Financial Services Register number 800542). Zopa Bank Limited is authorised by the Prudential Regulation Authority and regulated by the Financial Conduct Authority and the Prudential Regulation Authority. Simplyhealth Funding Services Limited may receive a commission from Zopa Bank Limited for introducing customers. The amount of commission varies depending on the product and amount borrowed, and does not affect the interest rate or the amount payable by the customer.
